220 hours
- 18-45 yrs. old
- high school graduate
- medical certificate
- voter’s ID or certificate of resgitration
- photocopy of barangay clearance/cedula
- 2 pcs. 1×1 pictures
100 hours
- 18-55 yrs. old
- (at least) finished 2 yr-course or in 3rd year in college
- voter’s ID or certificate of resgitration
- photocopy of barangay clearance/cedula
- 2 pcs. 1×1 pictures
100 hours
- 18-55 yrs. old
- college graduate (medical related course ex. PT, NURSING, MEDICINE)
- voter’s ID or certificate of resgitration
- photocopy of barangay clearance/cedula
- 2 pcs. 1×1 pictures
HOW TO APPLY:
- Complete your requirements
- Go to the Center for Excellence, open on Weekdays from 8am to 12 Nn and 1pm to 5pm
- Fill out the application form
- Wait for the call for orientation
