Marikina Mobile Passporting 2012

The Marikina City Civil Registry or Local Civil Registry (LCR) is again organizing a mobile passporting for Marikina residents in coordination with the Department of Foreign Affairs (DFA). For the second (2nd) time, Marikina-LCR is partnering with SM City Marikina which will be the venue for the passporting.

Take note of the following details if you wanted your passport to be processed during this event:

Submission of requirements: June 9, 2012

IMPORTANT NOTE: Only first 400 with complete requirements will be entertained

Schedule of interview and other procedures such as biometrics, payment: June 30, 2012

I Requirements for 1st time Applicants I

• Fee of Php1200.00
• Personal Appearance
• Authenticated Birth Certificate (NSO copy)
• At least 2 proof of identity, supporting documents indicating full name, date and place of birth and citizenship (Ex. valid identification cards such as SSS, GSIS, school, company, driver’s license, and PRC)
• Voter’s affidavit (for late registered

• for Female
  • NSO copy of marriage contract (for married)
  • NSO copy of death certificate (for widow)
• for Annulled/Divorced
  • Certificate true copy of divorce paper
  • certified true copy of annulment decision
• for Minor (below 18 yrs. old)
  • Personal and appearance of mother
  
  
• for Minor and is Not Traveling with Parent
  • Original and photocopy of DSWD clearance
  • Affidavit of consent
• for Minor with Both parents Overseas
  • Affidavit of consent (authenticated from Philippine Embassy)
  • Special Power of Attorney (SPA) (authenticated from Philippine Embassy)
  • Passport of traveling companion
  • Identification card of parents and traveling companion

I Requirements for Passport Renewal I

• Fee of Php1200.00
• Personal Appearance
• Old passport (original)
• Photocopy of 1st and last page of old passport
• Photocopy of latest arrival and departure (if any)
• Any document or ID with complete middle name

I For Passports Damaged during Typhoon Ondoy (Intact but Damaged) I

• Affidavit of mutilation
• Damaged passport
• Requirements as New Applicants

I For Lost Expired Passports I

• Affidavit of loss
• Requirements as New Applicants

Note: Lost Unexpired passports will not be processed in the mobile passporting, proceed to DFA Main Office for processing.
